ABSTRACT

In China, the current standard for cosmetic adverse reactions related skin disease (GB/T 17149.1-1997) was jointly issued by the Ministry of Health and the State Bureau of Technical Supervision in 1997, cosmetic-allergic adverse reactions include allergic contact dermatitis and photo-allergic contact dermatitis according to this standard. The increasing use and changes in cosmetic ingredients or formula lead to a significant increase for the incidence of adverse reactions as the cosmetics industry is developing rapidly in the last 20 years. In the meantime, the clinical manifestations have become more diverse. In recent years, there have been many reports on the special manifestations for cosmetic allergy and allergen test, which provide a reference for the subsequent improvement of the diagnosis and prevention.

Abstract Background: Cosmetics are part of the daily life of the population, and their use can lead to allergic contact dermatitis. Objectives: To assess the profile of patients diagnosed with allergic contact dermatitis to cosmetics treated at a referral center for 13 years, as well as the characteristics of the clinical picture and allergens involved. Methods: This was a retrospective study, with analysis of medical records of patients attended at this service. The individuals included had a diagnostic hypothesis of allergic contact dermatitis to cosmetics and had previously been submitted to epicutaneous tests. Results: A total of 1405 medical records were analyzed, 403 (28.7%) with suspected allergic contact dermatitis to cosmetics and 232 (16.5%) with confirmed diagnosis. Of these, 208 (89.7%) were women, and the age group most affected was 31 − 60 years. The most common locations were face in 195 cases (25.8%), cervical region in 116 (15.3%), and trunk in 96 (12.6%). The main allergens in the contact tests were toluene-sulfonamide-formaldehyde resin in 69 cases (29.7%), paraphenylenediamine in 54 (26.3%), Kathon CG® in 41 (20.7%), and fragrance-mix 1 in 29 (16.4%). In 154 (66.4%) of the 232 patients with a confirmed diagnosis of allergic contact dermatitis to cosmetics it was possible to specify the cosmetic product responsible for the lesions. Study limitations: The absence of some allergens considered important in the world as causes of allergic contact dermatitis, which are not readily accessible among us. Conclusions: The data of the analyzed population (predominance of young women), as well as the location of the lesions (face and cervical area) and the main allergens involved were consistent with those from the world literature.

ABSTRACT Objective: To evaluate the impact of pediatric asthma on patients of a specialized outpatient clinic in Southern Brazil. Methods: The study included children aged 8 to 17 years old with asthma diagnosis (mild, moderate and severe) under treatment at the asthma clinic of Hospital São Lucas da Pontifícia Universidade Católica do Rio Grande do Sul (PUCRS), Brazil. Measurements of spirometry, quality of life, disease control and atopy tests were applied. Results: A total of 66 children were included in the study and divided into groups, according to the severity of the disease: mild, moderate or severe asthma. The results showed similarities in both the treatment and the impact of asthma between groups, except for adherence to treatment: the group with mild asthma showed least adherence to treatment, and the group with severe asthma, greater adherence (p=0.011). As to school absenteeism, the group with severe asthma showed higher frequency (p=0.012), with over 10 days per year (p=0.043). Spirometry showed lower volume/capacity for the group with moderate asthma, followed by the groups with severe and mild asthma. All groups had a high prevalence of allergic asthma, with mites as the main allergens. For quality of life (QOL), and health-related quality of life (HRQOL) levels, there were no differences between groups. In addition, the values were close to the acceptable levels for the total score and for each one of the six domains. The same occurred for the HRQOL-asthma module. Conclusions: QOL and HRQOL present acceptable levels regardless of the severity of the disease.


RESUMO Objetivo: Avaliar o impacto da asma pediátrica de pacientes em acompanhamento ambulatorial em um centro de referência em pneumopediatria do Sul do Brasil. Métodos: Participaram do estudo crianças com idade entre oito e 17 anos, com diagnóstico de asma (leve, moderada e grave), em acompanhamento no ambulatório de asma do Hospital São Lucas da Pontifícia Universidade Católica do Rio Grande do Sul (PUCRS). Foram verificadas medidas de espirometria, avaliação dos níveis de qualidade de vida, controle da doença e teste de atopia. Resultados: Sessenta e seis crianças participaram do estudo, divididas em três grupos (asma leve, moderada e grave). Evidenciaram-se semelhanças tanto no tratamento quanto no impacto da asma, exceto para a adesão ao tratamento (p=0,011), em que o grupo de asma leve é o que menos adere e o grupo de asma grave o que mais adere ao tratamento. Em relação ao absenteísmo escolar, o grupo de asma grave apresentou o maior valor (p=0,012), com mais de dez dias/ano (p=0,043). As espirometrias demonstram menor volume/capacidade para o grupo de asma moderada, seguido do grupo de asma grave e asma leve. Os grupos possuem alta prevalência de asma alérgica, tendo os ácaros como os principais alérgenos. Quanto à qualidade de vida (QV) e à qualidade de vida relacionada à saúde (QVRS), não houve diferença entre os grupos. Além disso, os valores apresentados estão próximos aos níveis aceitáveis, tanto para o escore total quanto para os seis domínios analisados. O mesmo ocorre para o módulo QVRS-asma. Conclusões: Os níveis de QV e de QVRS demonstram-se aceitáveis, independentemente da gravidade da doença.

ABSTRACT

En los últimos años hubo un aumento significativo en la prevalencia de las enfermedades alérgicas pese a los avances en la comprensión de la patogénesis, la divulgación de guías para su control y tratamiento y la aparición de nuevos fármacos. La raz ón para este aumento no está totalmente estable cida, pero se considera que múltiples factores ambientales podrían estar involucrados en ello. El aire inspirado contiene numerosos agentes nocivos además de alérgenos ambientales; el asma y la rinitis alérgica son las principales expresiones clínicas respiratorias inmediatas posteriores a su inhalación. En la antropósfera, el entorno de la superficie terrestre habitada por los humanos, se han alterado los equilibrios naturales por la emisión de múltiples sustancias y se ha producido un creciente cambio climático. Este fenómeno global influye en la calidad del aire y consecuentemente en el desarrollo de enfermedades respiratorias. Dado que la bibliografía sobre el tema del control ambiental es muy amplia, y en ocasiones difícil de interpretar para poder realizar indicaciones precisas, válidas y sencillas de cumplir por parte de los pacientes, cuatro sociedades científicas de la República Argentina, dedicadas a este tipo de enfermedades, elaboraron un documento con información de fácil acceso a todo profesional médico que trate asma y/o rinitis, que expone medidas prácticas para los enfermos y alerta a los distintos actores involucrados en la salud pública acerca de las necesidades insatisfechas en este tema tan complejo, a fin de poder elaborar una agenda para su posible resolución.


In recent years there has been a significant increase in the prevalence of allergic diseases despite advances in the understanding of the pathogenesis, the dissemination of guidelines for its management and the emergence of new drugs. The reasons for this increase are not fully established, but it is suggested that multiple environmental factors may be involved. Inhaled air contains numerous harmful agents in addition to environmental allergens. The main immediate respiratory clinical expression after inhaling this contaminated air is asthma and rhinitis. The activity of human beings has altered the outdoor environment by the emission of multiple pollutants and has produced an increasing climate change. It also has a notable impact on the development of respiratory pathology and the modification of air quality. The bibliography on the subject of environmental control is very broad and sometimes difficult to interpret. In order to be able to make precise, valid and simple indications for patients to accomplish with, four scientific societies of the Argentine Republic that deal with this type of diseases, have elaborated a document that contains information of easy access to all medical personal involved in the treatment of patients with asthma and / or rhinitis, that provides practical measures for the patients and the different public health systems about unmet needs in this complex issue.

Abstract: Background: In our country, the Brazilian Standard Series is the most used for the etiological diagnosis of allergic contact dermatitis to shoes. However, there is no assessment of the usefulness of specific allergens for shoes. Objectives: To measure the improvement in diagnostic accuracy of allergic contact dermatitis to shoes with the use of a specific complementary series in patch testing and describe the characteristics of the affected population, such as gender, location of lesions, time of evolution, and the most common allergens. Methods: This retrospective study evaluated the results of 52 patients with suspected shoe dermatitis subjected to patch tests with the standard and specific series to quantify the gain in diagnostic accuracy. Results: Among the 52 suspected cases, 29 cases (56%) were confirmed. In 13 (45%) cases the diagnosis was determined through the specific series, which results in an 81% increase in the number of diagnoses. Study limitation: Small sample size. Conclusions: Women were more commonly affected, with a mean time for the final diagnosis of 45 months, and the most common localization was the dorsum of the feet. There was an increase in diagnostic accuracy with the introduction of new haptens in the patch test of patients with suspected shoes dermatitis.

ABSTRACT

Abstract: Children's products are considered safe by the general population and doctors. Labels with terms such as "hypoallergenic" or "dermatologically recommended and tested" denote trust and credibility with the idea that they can be used by any individual. Patients with allergic contact dermatitis may be sensitive to allergens present in any product, including children's. There is insufficient knowledge about allergens in these products in our country. We evaluated 254 children's products, and at least one allergen was present in 236 (93%) of them. The indication of a topical product should be careful and based on contact tests.

Abstract: Background: Metals are common agents of allergic contact dermatitis, occupational or not, with decreasing incidence over the last years in some countries that have regulated the amount of nickel in objects. Objectives: To analyze and compare with previous studies the profile of metal sensitization between 2003-2015. Methods: Patients who underwent patch testing between 2003-2015 were evaluated retrospectively regarding the sensitization rates to metals, the associations between them, the relationship with profession and epidemiology. Results: Of the 1,386 patients tested, 438 (32%) had positive test to some metal, similar results to the 404/1,208 (33%) of the previous study (1995-2002) performed at the same service (p=0.32). The frequency of nickel (77%), cobalt (32%) and chromium (29%) changed slightly (p=0.20). Most cases of sensitization to chromium were related to the occupation (64%), in contrast to nickel and cobalt (p<0.0001). There was a predominance of females among those sensitized to metal in both studies (p=0.63) and the age group of 20-49 years old (p=0.11); the number of fair-skinned individuals increased (p<0.001), as well as the lesions in the cephalic segment (50.5%; p<0.0001) and hands (45%; p<0.0001), which are not the most frequent location anymore. The number of cleaners decreased (39% vs. 59%; p<0.0001), which still lead in front of bricklayers/painters, which increased (14% vs. 9%; p=0.013). The frequency of wet work reduced (65% vs. 81%; p<0.0001). Study limitations: The study included a single population group; only patients with positive tests to metals were considered - the others were not evaluated for the possibility of false negatives. Conclusion: The sensitization to metals, occupational or not, has been significant over the last 21 years, with few epidemiological changes.

Contexte. Environ 80% des asthmatiques sont atopiques. La sensibilisation aux allergènes communs présente certaines variabilités environnementales et géographiques. Objectif. Déterminer le profil de sensibilisation aux allergènes communs des asthmatiques adultes de la ville de Kinshasa. Méthodes. De juin 2017 à février 2018, 216 asthmatiques de 18 ans et plus, des 2 sexes, ont été consécutivement recrutés aux Cliniques Universitaires de Kinshasa et dans certaines paroisses et églises de réveil de la ville de Kinshasa. A l'aide d'un questionnaire validé, les variables sociodémographiques ont été précisées. Le prick-test a été réalisé avec cinq extraits commerciaux standardisés de phanères de chien et chat, l'acarien de poussière de maison (Blomia tropicalis, Bt), les moisissures (Alternaria alternata), et le jaune d'œuf. Résultats. L'âge moyen de la population était de 45,23 (ET=17,56) ans, 74% de sexe féminin, 47 % non sensibilisés et 53% sensibilisés à au moins un allergène. Vingt-cinq pourcent étaient monosensibilisées et 27% plurisensibilisées. Le profil de sensibilisation était Blomia tropicalis (72%), phanères de chat (46%), phanères de chien (34%), Alternaria alternata (13%) et jaune d'œuf (11%). Conclusion. Une plurisensibilisation aux allergènes communs chez les asthmatiques dans notre milieu est présente dont le profil dominé par les acariens et les phanères de chats. Des enquêtes futures incluant un plus grand nombre de sujets et recourant à des batteries de tests plus élargies s'imposent en vue d'une définition d'options diagnostiques et thérapeutiques dans notre contexte

The association between pet ownership and the development of allergic and respiratory diseases has been the aim of several studies, however, the effects of exposure in adults remain uncertain. The aims of the present study were to investigate the prevalence of asthma and lung function status among dog and cat owners. This cross-sectional study was performed at two universities with students and workers who were allocated into 3 groups according to pet ownership in the previous year: cat owners, dog owners, and no pets (control group). Subjects underwent spirometry, bronchial challenge test with mannitol, skin prick tests, and questionnaires about animal exposures and respiratory symptoms. Control group comprised 125 subjects; cat owner group, 51 subjects; and dog owner group, 140 subjects. Cat owners had increased asthma prevalence (defined by symptoms and positive bronchial challenge test), but no changes in lung function compared to the control group. The dog owner group had lower spirometry values (forced expiratory volume in one second and lower forced vital capacity), but similar asthma prevalence, compared to the control group. In the cat owner group, excess of asthma may have an immunological basis, since we found an association with atopy. Although we did not have endotoxin data from volunteers' households, we postulated that low values of lung function were associated to exposure to endotoxins present in environments exposed to dogs.

ABSTRACT

ABSTRACT Cats are a significant source of allergens that contribute towards worsening of allergic diseases. The aim of this study was to investigate the association between sensitization to cat allergens and allergic respiratory diseases.This was an observational retrospective study based on the skin pricktests results of patients at a tertiary-level hospital in São Paulo. A total of 1,985 test results were assessed. The prevalence of sensitization to cat allergen was 20% (399 patients). Our data indicated that in this population of atopic patients, a positive skin prick test result for cat allergen was not associated significantly with a diagnosis of respiratory allergy.

A asma e a rinite alérgica são doenças frequentes e acometem parcela significativa da população, sobretudo crianças. Frequentemente a asma e a rinite coexistem e tem sido documentado que a presença de rinite potencialmente aumenta a gravidade da asma e impacta negativamente na qualidade de vida. Entre os agentes desencadeantes/agravantes dessas doenças são apontados: aeroalérgenos (ácaros do pó domiciliar, fungos, alérgenos de baratas, epitélio de animais, polens e ocupacionais), poluentes intradomiciliares e extradomiciliares (fumaça de tabaco, material particulado liberado pela cocção/aquecimento ­ gás de cozinha, fogão a lenha) e irritantes (odores fortes, arcondicionado). O objetivo desse estudo foi identificar as medidas recomendadas para reduzir a exposição de pacientes sensíveis a esses agentes. Realizou-se busca em base de dados MEDLINE, SciELO e LILACS empregando-se os descritores: environmental control, mite, cockroach, fungi, furry pets, pollen, irritants, smoking, indoor pollution, cooking. Foram revisados os principais estudos e elaborou-se um documento em que são discutidas as relações entre exposição e aparecimento de sintomas, assim como as medidas apontadas como tendo potencial para evitar a exacerbação/ agravamento das doenças alérgicas respiratórias.


Asthma and allergic rhinitis are highly prevalent diseases and they affect a significant share of the population, especially children. Very often, asthma and rhinitis coexist, and the presence of rhinitis has been shown to potentially increase the severity of asthma, with a negative impact on quality of life. Among the triggering or aggravating agents of these conditions it is possible to list: aeroallergens (house dust mites, fungi, cockroach allergens, animal epithelium, pollens and occupational allergens), indoor and outdoor pollutants (tobacco smoke, particulate matter released by cooking/heating ­ cooking gas, wood stoves), and irritants (strong odors, air conditioning). The aim of this study was to identify measures recommended to reduce the exposure of patients sensitive to these agents. A search was conducted on the MEDLINE, SciELO, and LILACS databases, using the following keywords: environmental control, mite, cockroach, fungi, furry pets, pollen, irritants, smoking, indoor pollution, cooking. The main studies were reviewed, and a report was prepared in which the relationships between exposure and the onset of symptoms are discussed, and measures with a potential to prevent exacerbation/ aggravation of allergic respiratory diseases are presented.

ABSTRACT

Food-dependent exercise-induced anaphylaxis [FDEIA] is one of the severe allergic reactions in which symptoms develop only if exercise takes place within a few hours of eating a specific food. It is important to consider FDEIA in cases of unexplained anaphylaxis as reactions can occur several hours after ingesting the culprit food [s]. We herein report the first two cases of FDEIA in the Middle East. The first one is induced by wheat, while the other by peanut. The pathophysiology, predisposing factors, diagnosis, and treatment of FDEIA are also summarized here

A asma é um importante problema de Saúde Pública. Estima-se que ela comprometa cerca de 300 milhões de pessoas e mate 250 mil a cada ano, em todo o mundo. Trata-se de uma alteração inflamatória crônica das vias aéreas, cujas principais características incluem, também, grau variável de obstrução ao fluxo aéreo e hiper-responsividade brônquica. O tratamento é baseado em fármacos anti-inflamatórios e broncodilatadores. Para pacientes que permanecem sintomáticos vêm sendo desenvolvidas novas terapias, desenhadas para atingir alvos-chaves na patogenia.


Asthma is a serious health problem throughout the world. It is estimated that 300 million people are affected and 250 thousands are killed by asthma worldwide. Asthma is a multifactorial chronic inflammatory disorder of the airway in which the chief features include a variable degree of airflow obstruction and bronchial hyper-responsiveness, in addition to the underlying chronic airway inflammation. Asthma's treatment is based on anti-inflammatory and bronchodilator drugs. For patients who remain symptomatic new therapies tailored to target key pathways in asthma pathology are being developed.

BACKGROUND: Contact dermatitis to cosmetics is a common dermatosis, especially in adults and professionals who handle them. OBJECTIVE: The objective of this study was to evaluate the frequency of sensitization to cosmetics' components in patients with suspected allergic contact dermatitis and to identify the main sensitizers related to occupational contact dermatitis. METHODS: During the period of January 2008 to June 2010, all the patients with a presumptive diagnosis of allergic contact dermatitis to cosmetics were selected. The patients were submitted to the patch tests of cosmetics series, composed by ten substances. RESULTS: Among the 147 patients studied sensitization to cosmetics components occurred in 31,29% of the cases, 14 of those (19,18%) equally corresponding to BHT and triethanolamine substances, 13 (17,81%) to ammonium thioglycolate, 09 to sorbic acid (12,33%), 08 to tosilamida (10,95%), 06 to germall (8,22%). The other elements tested showed indices of 5% or less. A higher frequency of contact dermatitis to cosmetics was observed in women and the age most affected was concordant with the age range of greatest professional activity of the population. CONCLUSION: Allergic contact dermatitis was more frequently associated with Triethanolamine, BHT and ammonium thioglycolate, and the relation with occupational contact dermatitis was discreet.


FUNDAMENTOS: A dermatite de contato por cosméticos é uma dermatose relativamente comum, sobretudo em adultos e em profissionais que os manipulam. OBJETIVO: O objetivo do estudo foi avaliar a freqüência de positividade aos componentes de cosméticos, em pacientes com suspeita de dermatite alérgica; e identificar os principais sensibilizantes relacionados à dermatite de contato ocupacional. MÉTODOS: Durante o período de janeiro de 2008 a junho de 2010 foram selecionados todos os pacientes com hipótese de dermatite alérgica de contato a cosméticos. Os pacientes foram submetidos aos testes epicutâneos com bateria de cosméticos, composta por dez substâncias. RESULTADOS: Dos 147 pacientes estudados, a sensibilização aos componentes do cosmético ocorreu em 31,29% dos casos, sendo 14 (19,18%) corresponderam igualmente às substâncias BHT e trietanolamina, 13 (17,81%) ao tioglicolato de amônia, 09 ao ácido sórbico (12,33%), 08 tonsilamida (10,95%), 06 germal (8,22%). Os demais elementos testados proporcionaram índices iguais ou inferiores a 5%. Observou-se maior freqüência de dermatite de contato aos componentes da bateria de cosméticos em mulheres e a idade mais acometida foi concordante com a faixa etária de maior atividade profissional da população. CONCLUSÕES: Houve maior freqüência de dermatite alérgica de contato pelas substâncias trietanolamina, BHT e tioglicolato de amônia, demonstrando pouca associação à dermatite de contato ocupacional.

ABSTRACT

OBJETIVO: Verificar o impacto de uma intervenção educativa problematizadora sobre o controle ambiental para aeroalérgenos, conduzida por agentes comunitários de saúde, nos domicílios de crianças com asma. MÉTODOS: Estudo prospectivo de intervenção. Foram visitados 95 domicílios com crianças (faixa etária de 2-10 anos) com episódios recorrentes de dispneia e sibilância no município de Iguatu (CE). A intervenção educativa envolveu grupos focais com agentes comunitários de saúde com a problematização sobre medidas educativas a serem implementadas. Após a intervenção, 84 domicílios foram revisitados e reavaliados. RESULTADOS: Após a intervenção, houve uma redução significativa na limpeza do piso com vassoura (p = 0,02), no uso de fogão a carvão (p = 0,001) e no número de bichos de pelúcia no domicílio (p = 0,05). Houve uma melhoria significativa no controle ambiental para aeroalérgenos nos quartos das crianças após a intervenção (p = 0,003). CONCLUSÕES: A intervenção educativa mostrou ser uma importante ferramenta junto à população estudada, minimizando os aspectos negativos das condições ambientais dos domicílios das crianças asmáticas.


OBJECTIVE: To determine the impact that an educational intervention employing problem-solving strategies for the environmental control of aeroallergens has on environmental conditions in the households of children with asthma. The intervention was carried out by community health agents. METHODS: This was a prospective intervention study. The community health agents visited 95 households in which there were children (2-10 years of age) who had recurrent episodes of wheezing and dyspnea. All of the households investigated were in the city of Iguatu, Brazil. The educational interventions involved focus groups and community health agents. The focus groups were asked to brainstorm the educational measures to be implemented, in order to devise problem-solving strategies. After the intervention, 84 households were revisited and reevaluated. RESULTS: After the intervention, there was a significant decrease in the use of brooms for cleaning the floor (p = 0.02), in the use of coal-burning stoves (p = 0.001), and in the number of stuffed toys (p = 0.05). There was also a significant post-intervention improvement in the environmental control of aeroallergens in the bedrooms of the children (p = 0.003). Conclusions: The educational intervention applied in this study proved to be an important tool for the target population, improving environmental conditions in the households of children with asthma.

ABSTRACT

El asma bronquial constituye uno de los principales problemas de salud en el municipio de Nuevitas. Objetivo: determinar el comportamiento de aeroalergenos y factores medio ambientales en este municipio, durante el período comprendido entre el primero de octubre de 2002 hasta el treinta de septiembre de 2007. Método: se realizó un estudio descriptivo transversal y de ensayo comunitario medio ambiental. El registro primario estuvo constituido por la encuesta que contenía una serie de variables según la bibliografía revisada. Resultados: se encontró presencia de aeroalergenos, tales como hongos anemófilos, de los que predominó el l Aspergyllus con el 100%, sobre todo en el Consejo Casco – Zona Industrial que presentó la mayor cantidad de microvertederos. Conclusiones: se observó una gran carga ambiental de alergenos e irritantes que favorecen la aparición de las enfermedades alérgicas.


Bronchial asthma constitutes one of the main health problems in Nuevitas municipality. Objective: to determine the aeroallergens and environmental factors behavior in this municipality, from october 1st 2002 to september 30 2007. Method: a cross-sectional descriptive study and a community environmental assay were performed. The primary registration was constituted by a survey that contained a series of variables according to the revised bibliography. Results: aeroallergens were found, such as anemophilous fungus, of those that prevailed was the l Aspergyllus with 100%, mainly in the old town- Industrial Area that presented the biggest quantity in micro-dumping places. Conclusions: a great environmental load of allergens and irritants was observed that favor the appearance of the allergic diseases.

Introducción: La poliposis nasosinusal (PN) es una enfermedad inflamatoria crónica de la mucosa. Apoyado en los hallazgos clínicos, histológicos e inmunológicos, se postula a la alergia como un factor etiopatogénico, lo cual no ha sido plenamente demostrado. Objetivo: Evaluar la presencia de hipersensibilidad a aeroalergenos en pacientes con PN y compararla con pacientes normales. Material y método: Se realizó prick test a aeroalergenos a todos los pacientes utilizando 30 alérgenos frecuentes en el área metropolitana de Santiago, incluyendo pólenes de árboles, malezas y pastos, ácaros, epitelio de animales y hongos habituales. Este test se amplió con el uso de tres hongos habitualmente no evaluados (Stemphyllium, Pullularia, Helminthosporium). Se utilizó la prueba de Chi cuadrado con una significancia de 0,05. Resultados: El 71 por ciento de los pacientes con PN (n =73) y el 66 por ciento del grupo control (n =44) presentaban un test cutáneo positivo, sin diferencia estadística significativa. El patrón de sensibilización a aeroalergenos fue similar, salvo para los alérgenos fúngicos: los pacientes con PN estaban sensibilizados en 40 por ciento a Pullularia, 30 por ciento a Stemphyllium, y 10 por ciento a Helminthosporium. En tanto que los controles no presentaron sensibilización a estos hongos no habituales. Discusión y conclusiones: Existe una alta tasa de sensibilización a aeroalergenos en pacientes normales y con PN, lo que sugiere un probable rol de la alergia en la patogénesis de la poliposis nasal. Destaca un patrón diferente de sensibilización a alérgenos fúngicos, cuya relevancia clínica debe ser evaluada a futuro.


Introduction: Nasal polyposis (NP) is a chronic inflammatory disease that leads to tissue oedema and eventually polyps. The pathogenesis of NP has not been fully understood yet, but there are clinical, histological, and immunological findings that suggest that allergy plays a role. Aim: The aim of this study was to find out the prevalence of airborne allergen hypersensitivity in patients with nasal polyposis. Material and method: Skin prick test (SPT) was performed to patients with NP and to a control group (CRS). Thirty frequent airborne allergens of the Santiago de Chile metropolitan area, including tree, grass and weed pollens, house dust mites, animal dander and common fungal allergens were tested. This SPT was extended by using three fungal allergens that usually are not evaluated (Stemphyllium, Pullularia, Helminthosporium). Chi square test was used to compare both groups of patients. Results: 71 percent of NP patients (n=73) versus 66 percent of the controls (n=44) had a positive SPT, although there was no statistical significance. The only difference in sensitization was for fungal allergens. In NP patients, sensitization to Pullularia was about 40 percent>, 30 percent> to Stemphyllium, and 10 percent to Helminthosporium. CRS patients were not sensitized to uncommon fungal allergens. Conclusions: We found a high frequency of sensitization to airborne allergen in CRS and NP patients that suggest a probable role of allergic inflammation in the pathogenesis of nasal polyposis. Stands out a different pattern of sensitization to fungal allergens in the NP group; the clinical relevance of this finding awaits future evaluation.

ABSTRACT

A 25-year-old woman was diagnosed to have tubercular meningitis (TBM) with a right parietal infarct. She responded well to four-drug anti-tubercular treatment (ATT), systemic steroids and pyridoxine. Steroids were tapered off in one and a half months; she was put on two-drug ATT after two months. Six months after initial diagnosis she presented with sudden, bilateral visual loss. Vision was 3/200 with afferent pupillary defect and un-recordable field in the right eye; vision was 20/60 in the left eye, pupillary reaction was sluggish and the field showed a temporal hemianopia. On reintroduction of systemic corticosteroids vision improved (20/120 in right eye and 20/30 in left eye) within three days; the field defects improved sequentially to a left homonymous hemianopia, then a left homonymous inferior quadrantonopia. A diagnosis of TBM, on treatment, with bilateral optic neuritis, and right optic radiation involvement was made. Since the patient had been off ethambutol for four months, the optic neuritis and optic radiation lesion were attributed to a paradoxical reaction to tubercular allergen, corroborated by prompt recovery in response to corticosteroids. This is the first report of optic radiation involvement in a paradoxical reaction in neuro-tuberculosis in a young adult.

ABSTRACT

Existe un aumento en la prevalencia de las enfermedades alérgicas y el asma. Dicho incremento es más notable en la población urbana de los países occidentales más industrializados. Enfermedades como el eczema atópico, la alergia a alimentos, la rinoconjuntivitis alérgica y el asma bronquial, entre otras, además de generar costos millonarios a los distintos estados, afectan notablemente la calidad de vida de la población que las padece, reflejándose en un gran ausentismo escolar y laboral entre otros aspectos. Las enfermedades alérgicas y el asma surgen de la interacción de factores genéticos y factores ambientales. Dicha interacción que provocará la sensibilización atópica y posteriormente el desarrollo de las distintas enfermedades, comienza en la vida fetal intrauterina, siendo clave el primer, y tal vez el segundo año de vida. Se describen diversas estrategias de prevención primaria para evitar la sensibilización, entre las que se destacan, el control ambiental para evitar o disminuir el contacto a diversos aeroalérgenos interiores, la manipulación en la dieta fomentando lactancia materna exclusiva hasta los 4 a 6 meses, utilizando en niños de riesgo fórmulas hidrolizadas como suplemento, evitar el tabaquismo en la embarazada y el pasivo en los niños pequeños. Entre las estrategias en la prevención secundaria, es decir, el evitar el desarrollo de una enfermedad alérgica después de la sensibilización, se destaca nuevamente el control ambiental (se mencionan distintos consejos de la OMS), la inmunoterapia con alérgenos y la farmacoterapia (antihistamínicos). Por último, se mencionan la influencia de la polución ambiental y el potencial papel de la terapia génica.


There has been an increase in the prevalence of asthma and allergic diseases, more notably in urban population of western developed countries diseases such as atopic eczema, food allergy, allergic rhinoconjunctivitis and bronchial asthma, among other, not only generate millionaire costs to the different states, but also considerably affect the quality of life of the population suffering them, which is reflected in a great work and school absence, among other aspects. Allergic diseases and asthma develop from the interaction between genes and environment. Such an interaction will cause allergic sensitization and lately the development of different diseases. It starts in fetal life and becomes more important in the first, and perhaps second, year of life. Different strategies for primary prevention are described to avoid atopic sensitization, such as allergen avoidance of indoor allergens, diet manipulation to avoid cow's milk protein with exclusive breast feeding until four to six months of life; use of hydrolyzed milk formulae as a replacement for or supplement to breast-feeding, late introduction of solid food, avoidance of active smoking among pregnant women and passive smoking in children. Among other strategies for secondary prevention, this is to avoid the development of an allergic disease after sensitization has occurred, again allergen avoidance is mentioned (with advice measures by WHO), allergen immunotherapy and medication (antihistamines). At last, the influence of air pollution and the potential use of gene therapy are also considered.
